A LESSON TO SHARE

An incident occurred at one of our projects when a pin has fallen during a careless dismantling of a passenger hoist that has caused damage to a vehicle. But then, this has turned the incident into a traumatic tragedy for the passengers inside the mentioned vehicle caused by the extremely delayed response which should have been done immediately.

To our knowledge, the incident did not have any casualties and we focused our attention only on the material damaged. We were more focused on other agendas and did not see the tragedy to be understood very seriously.

We regret and we are truly sorry of how tragic and serious this experience towards the family of the passengers, mentally and psychologically, which was caused by the delay in responding to and the narrow minded action in handling this matter

We now truly understand and feel the non-material impact which has happened to an individual at the very moment and perhaps carried-on to the future.  Psychologically, we understand that this more than just an incident which has caused some mental damage where recovery may take a long time and difficult.

This tragedy has really opened up our eyes, heart and mind that the size of the incident does not matter and cannot be underestimated.  Any incident must be addressed immediately, for both material  and  non-material.

After the tragedy, all the involved parties, who are responsible along the whole process, have been reprimanded publicly: from the General Affair, Equipment Manager, Safety Manager, Project  Manager, up to  the Project Director.  Apology statements from the President Director have also been addressed in-person to the family member. 

This can be a valuable lesson for our other projects in the future to be more careful and provide a much quicker response. As a preventative measure, some reviews of the overall SOP have been conducted in order to seek the valuable improvement, towards a reliable safety process.

Most importantly, we truly hope that the family members can win back a healthy mental & mind and after this tragedy, there will be no other or similar incident that anyone has to go through.

This has been a great deal of a lesson to learn for us to be better and more conscious towards safety for the people around us as for our own family and the loved ones.
